What Agenda for Change is
Agenda for Change (AfC) replaced local NHS pay scales in 2004. It sets unified terms for pay, hours, unsocial enhancements, pension and annual leave for most NHS staff outside very senior manager and medical contracts. Jobs are evaluated against 16 factors and placed on bands 2 to 9.
How NHS pay bands work
Each band has spinal column pay points. You normally move up one point every two to three years until you reach the top of the band. Gross pay may include High Cost Area Supplement in London, unsocial hours enhancements and on-call payments. Pensionable pay usually includes basic salary and certain allowances.

England, Scotland, Wales and Northern Ireland
England and Wales share the same AfC cash scales and received a 3.3% uplift from 1 April 2026. Scotland has a separate multi-year deal with a 3.75% uplift in 2026/27. Northern Ireland follows the NHSPRB recommendation in cash terms. Scotland also uses a 36-hour reference week, so hourly rates differ from England even when annual salaries are similar.
Pension and leave under AfC
Most AfC staff belong to the NHS Pension Scheme. Member contributions are tiered on whole pensionable pay. Use the NHS pension contribution calculator for your tier.
